The Client Company must complete an election form to offer this benefit to the worksite employees. Without this election you may not participate. You will need to check with your supervisor to see if this benefit is available at your location.

If offered at the Client worksite location, every eligible employee will enroll in the basic term life insurance of $15,000 term life with AD&D. The cost of the basic term life if paid by the Client. You will then have the option of purchasing additional term life insurance as noted in other areas of this section.
